School Health Services
WELCOME TO THE STUDENT HEALTH DEPARTMENT
Canby School District provides school health services for students. Our District nursing team collaborates with each school as well as the district to provide individualized health plans and district-wide trainings for staff.
The nursing team also attends parent meetings and acts as a liaison between the district and the physician's office to gather necessary medical information. Health Services to students are provided in a variety of ways, depending on student need.
Immunizations
The Oregon Health Authority requires all children to have up-to-date immunizations or complete the exemption process by a certain date each year to stay enrolled in a public or private school. For the 2023-2024 school year, that date was Feb. 21.
Health Management Plans
There are students who require individualized Health Management Plans in order to be safe at school. These plans are written by our district nurses in collaboration with the parent and the treating physician. They are distributed to appropriate school staff as necessary for safety. Plans are reviewed/updated at least annually.

Medication Forms
For students who will need medication administered at school, parents need to complete a medication authorization form either at the school, or using our fillable forms. Please see below. The medication must be in the original container. Prescription medication must be in the original container with a current prescription label. All staff who administer medication have been trained by our district nurses.

Hearing and Vision Screenings
Canby School District conducts annual hearing screenings in our primary grades or based on teacher referral if there is a concern. Please contact your child's teacher if you have a concern about your child's hearing.
Canby School District works with the Oregon Lions Club to provide free vision screenings each year as well at all of our elementary schools (grades K-6), plus Ninety-One School (grades K-8).
The screening is painless and is done from a distance, not touching the child’s eyes or face. Most children pass the exam, and those results are not sent home, except by request. For those children whose no-passing result warrants follow-up with a specialist or health-care provider, the details of the screening will be sent to parents or guardians.
